Request Records

The Superintendent is the Freedom of Information Coordinator at Thornapple Kellogg Schools, and his office is responsible for handling FOIA requests. Written FOIA requests should be sent to the attention of Superintendent of Schools, Thornapple Kellogg Schools, 10051 Green Lake Road, Middleville  MI  49333.  Requests can also be faxed to the office at (269) 795-5401 or e-mailed to [email protected].
 
The District has prepared a FOIA request form available here to be used to submit requests.
