Nominations must include the nominee's name, address, phone number and a brief explanation - about 250 words - of why the nominee should be considered our community's Hometown Hero for 2016. Nominators are asked to include their contact information as well.

The 2016 Hometown Hero will be recognized at the annual Hometown Hero Dinner put on by the TAEF in September.
